Hi! I would like to file a financial assistance for the period of my sickness due COVID-19. Hoping your assistance. Thank you.

I was going to work last July 30, 2021 and I suspect the driver had covid19. He coughed and spat as well.
Aug 3 - I got fever
Aug 4 - got my swab RT-PCR test
Aug 5 - got the result - positive
Aug 8 - they bring me to quarantine facility in National Care for Mental Health
Aug 17 - discharged from the Hospital
Aug 18 to 19 home quarantine as per Doctors Advice

Your Request

You asked for ECC Claim Form for COVID19 Positive.

Response to Request

Your FOI request is approved. 

If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

The forms are available at the SSS/GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs

Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS/GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs

Your right to request a review
