Hi! I would like to get an application for ECC financial assistance on Covid-19. I just learned today about the benefits for covid-19 patient cash assistance. I was tested positive since Aug. 24 and took 14 days home quarantine that ended in September 6. I'm a government employee and PWD. Can I still avail the benefit of P30K even it is past the 14 days quarantine? If so, may I know what are the requirements or needed to avail this benefit.

Your Request

You asked for COVID-19 Financial Assistance.

Response to Request

    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs

Your right to request a review

Hello! Thanks for the response.

I just visited the website of GSIS and ECC but can seem to find the specific form. I browsed the list but don't know which one. I'd like to know the specific name of form so I can easily look for it? Or can you give me the form here so I can directly download the form from you.
