Hi Sir/Ma'am,

Good day!

I am asking financial assistance to your good office because I am a no work no pay employee.  I was tested positive with Covid-19 last August 18, 2021.  I was quarantined from August August 12, 2021 to August 25, 2021.

Greetings!

Thank you for your request dated Sep 07, 2021 08:24:23 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Financial Assistance for Covid-19 positive.

Response to Request

    If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

    The forms are available at the SSS/GSIS/ECC website.

    EC Claims Requirements:
    1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
    2. RT-PCR or Swab Test Result (Positive)
    3. Medical Certificate or Home Quarantine Clearance
    4. 2 Valid IDs

    Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

    If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

    The EC CA application form is available in our FB page and the requirements are as follows:
    1. Proof of Approved EC Claim Voucher from SSS/GSIS
    2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
    3. 2 Valid IDs

Your right to request a review
