Hi! Magandang Hapon po.

I would like to request for ECC claim for financial assistance under  compensable diseases. 

I am covid 19 positive. I've started to isolate last August 14, and then I got swabbed last August 18 resulting positive. I'm currently on my 4th day of home quarantine based on BHERT/LGU Home quarantine Monitoring. According to them, I can get my Home quarantine clearance on August 31, so I need to stay at home for the mean time without my daily income.

I am a Customer Assistant at SM Savemore Market which is I am exposed to different customers.

Your Request

You asked for ECC CLAIM FOR COVID POSTIVE PATIENT.

Response to Request

Your FOI request is approved. 

If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

The forms are available at the SSS/GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs

Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS/GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs

Your right to request a review
