Hello SSS and ECC,

I am writing to seek for assistance and approval for my sickness and cash assistance since I was positive for Covid 19 with confirmatory from Aug 18 tests taken in Makati Medical Center.  Symptoms  started August 12 and I am on quarantine as advised by LGU until Aug. 30. I am working in a BPO hired last May 24 of this year for the reason I only have limited number of leaves ( SL and VL ). If you need anything else please feel free to reply on this or contact on my personal number.

Greetings!

Your Request

You asked for Covid-19 Positive - Cash Assistance.

Response to Request

Your FOI request is approved. 

If you are an employee who suffered from a work-connected COVID,  pls file your EC sickness claim at the SSS branch nearest you(for private sector employees);  or at the GSIS (for public sector employees).

The forms are available at the SSS/GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs

Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)

If you already have an approved EC sickness claim at SSS/GSIS,  you can then file an EC cash assistance claim at the ECC.

The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS/GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs

Your right to request a review
