Hi! I would like to follow up my Vax Cert Request with:

To whom it may concern: 

This is to bring to your attention that I have been trying to access my vaccine certificate numerous times since last week, January 29, 2022. 

However, your system always declines it and always tells me that there is no vaccination record found. I have already submitted the copy of my vaccination card and my passport in your system, 

I am really hoping you can help me

While our aim is to provide information whenever possible, in this instance this Office (GUIMBA WATER DISTRICT) does not issue a vaxcert.
