Hello. I would like to request for a copy of my Form137A. I am applying to study abroad and the school requires this document. The high school that I graduated from (Isabela State University Science and Technology High School Garita Heights Cabagan) cannot provide a copy as all documents in their registrar's office was destroyed by fire in 2005. So I kindly ask for your assistance in this. Thank you.
