Dear FOI,

I have been following up with DOH on their email and I tried to call the landline but I am not able to connect. I would like to have my Vaxcert updated in the system as my second dose is not recorded. I will be needing the vaxcert to travel abroad as I am not able to obtain an earlier appointment from BOQ. 

Below is my vaccination details:

First dose( Pfizer) - 10/22/2021
Second dose( Pfizer)- 11/12/2021 ( no record)
