Sir and Madame:
I would like to request a copy/print out of the above OCT I-2062; OCT P-5195, Certificate of Transfer of Title No. 15040 all in the name of JULIAN GALAMAY, Lot PSD 16686 located in Barrio San Isidro ( Now known as MINANTE 2),  Cauayan, Isabela  He was our late grandfather. I visited the LRA and RD of Isabela and Tuguegarao (Region 02) but was referred and instructed to follow up in Quezon City. I followed on line but since all the documents were all in the late 30's and 60's I believed its all in your archives/microfilm. Thank you. S. Galamay.

You asked for OCT I-2062; OCT P-5195; (JULIAN GALAMAY); Cert. T-15040( JULIAN GAL AMAY).

Response to Request

If you would want to have a copy of the concerned title number/s, it can be requested online through eSerbisyo or at the nearest Register of Deeds (anywhere-to-anywhere transaction) or at One-Stop-Shop, LRA Central Office, window 3.

Please provide the following details:

1. Title number
2. Registered Owner
3. Location of the property

Since the location of the property is in Isabela, possible that the copy of the title was destroyed by fire. Please request a certification indicating that the said title number has no more record in the concerned Registry of Deeds then comply with the needed requirements for reconstitution.
