Issuance of the Owner's Duplicate Copy of Title.

Please refer to the following requirements:
1. Affidavit of Loss
2. Photocopy or Title Certified by The Registry of Deeds (latest)
3. Signed Copy of the Petition
4. Latest Tax Declaration
5. Latest Tax Receipt

Kindly ask for assistance from the Court that has jurisdiction over your property if there will be an additional requirement/s for the said request.
