Goodafternoon. This is to kindly request from your good office for the APPROVED SURVEY PLAN & LAND DESCRIPTION for TCT No. 1139 with the following details :

Spouse Name : TEODORA TUAZON
Lot Situation : SAN BARTOLOME, MUNICIPALITY OF STO.TOMAS, PAMPANGA
Lot Area : 37,793 sq.mtrs. 
DAR Emancipation Patent No. : 124229

Please note that we've been in DAR & DENR R3 to ask the same documents, however, advised us that above TCT approved survey plan was being filed and submitted to LRC in East Ave., QC.

Your concern can only be requested by walk-in or in person at LRA One-Stop-Shop windows 7 and 8. In your case, kindly ask someone to request it for you (SPA is not necessary).
