I Sarah Cabayao-Bacerra, one of Cabayao heirs, want to request the following documents for the purpose of property survey reference.

1. Copy of Original Certificate of Title of no. 36116
2. Blueprint copy of the same property with OCT no. 36116
3. Copy of TCT no. T-180293 (dated Feb 18, 2000) of Lot 61-B

If you would want to verify and/or secure a copy of the concerned title, it can be requested online through eSerbisyo or at the nearest Register of Deeds (anywhere-to-anywhere transaction) or at window 3 of the Land Registration Authority One-Stop Shop located at East Avenue corner NIA Road, Diliman, Quezon City

Please provide the following details:

1. Title number
2. Registered Owner
3. Location of the property

For plan copy concerns if it was approved by LRA, it be requested by walk-in or in person only at windows 7 and 8 of the Land Registration Authority One-Stop Shop located at East Avenue corner NIA Road, Diliman, Quezon City. In case you can't request it in person, kindly ask someone to request it for you (SPA is not necessary) if not, request it from the Regional Office of DENR where the property is located.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by sending an e-mail to our Public Relations and Information Section at pris@lra.gov.ph.

Your review request should explain why you are dissatisfied with this response and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
