Good day sir/ma'am. I would to request the said copy for determination of technical description as part of our lot survey. The geodetic engineer ask for it since according to the heirs they lost the original. Gregorio Dela Cruz Sr/ Concordia Dela Cruz, Lot is located at Brgy Kanantong Kiamba Sarangani Province.

Response to Request

If you would want to verify and/or secure a copy of the concerned title, it can be requested online through eSerbisyo or at the nearest Registry of Deeds (anywhere-to-anywhere transaction) or at window 3 of the Land Registration Authority One-Stop Shop located at East Avenue corner NIA Road, Diliman, Quezon City

Please provide the following details:

1. Title number
2. Registered Owner
3. Location of the property

Please note that our office can only trace and verify by title number, not by lot, name, or tax declaration number.

Since the Owner's Duplicate is not intact anymore, you may file a request for Issuance of the Owner's Duplicate Copy of the Title
(Only if there is still a Registry of Deeds Copy. If none, file for Judicial Reconstitution)

Please refer to the following requirements:
1. Affidavit of Loss (to be annotated at the Registry of Deeds title copy)
2. Photocopy or Title Certified by The Registry of Deeds (latest)
3. Signed Copy of the Petition
4. Latest Tax Declaration
5. Latest Tax Receipt

Kindly ask for assistance from the Court that has jurisdiction over your property if there will be an additional requirement/s for the said request.

Your right to request a review
