Dear Sir/Madam,

Good day! I am Maria Innah Borres,  of legal age & a resident of Banica Roxas City, Capiz. I am writing this letter to your good office and would like to request for a certified true copy of Subdivision Plan of LOT 1-A-2 (LRA) PSD-401523  with Urban Code 061914 containing an area of 154 sq. m. located at Barangay Banica, Roxas City, Capiz. This is to have a copy in reference to verifying the location of our Lot and to know what is the exact right of way in our area. Thank you and hoping for a positive response regarding this matter. Your concern can be requested by walk-in or in person only at LRA One-Stop-Shop windows 7 and 8. In case you can't request it in person, kindly ask someone to request it for you (SPA is not necessary).

Your right to request a review
