Hi!

I am Teddy John C. Mañale from LTO DLRC Limketkai Center and I would like to confirm an LTO Memorandum Order 2019-003 issued on November 22, 2019 exists. The said memorandum order prescribes a mandatory phrase that must be included in an Affidavit of Loss as a requirement for Duplicate DL application. 

The phrase is:
"That the Driver's License has not been confiscated by any Law Enforcement Agency or has no pending or unresolved case for Apprehension, Alarm, Suspension or Revocation"
 
If the said memorandum order was issued, we request a copy to  be used by our office.
