Hi my name is Nash Andrei P. Ducot, i would like to request my driver's license serial number with the Card Number: C02-17-013849. My card was lost last december 7, 2021 during our trip in Zambales. I solemnly swear that i would only use this information for the renewal of my driver's license that will expire on February 14, 2022. Thank you!

Response to Request

The Serial Number is required in creating an LTMS Account. Unfortunately some of our clients still don't have a Driver's License Card. So, in Creating your LTMS Account, you will see the first question will be: 
" Do you have a Philippine Driver’s or Conductor’s License?"

Kindly select "NO" and then proceed to complete the Client Information.
