I would like to ask about bicycle incident here in Negros Oriental. Please, include the list below:

1. address of the cyclist 
2. time and date of the incidents occurred 
3. age of the cyclist 
4. gender of the cyclist
5. where did the accident happened
6. what is the cause of the incident
7. collision type
8. Severity (Fatal Injury, property damage)
