Hi!

I am Teddy John C. Mañale from LTO DLRC Limketkai Center and I would like to request for a copy of an LTO Memorandum Order 2019-003 issued on November 22, 2019. The said memorandum order prescribes a mandatory phrase that must be included in an Affidavit of Loss as a requirement for Duplicate DL application. 

The phrase is:
"That the Driver's License has not been confiscated by any Law Enforcement Agency or has no pending or unresolved case for Apprehension, Alarm, Suspension or Revocation"
 
The document will be used for our office's copy and whenever client will ask for the said memo.
