Hi. This is in relation to my request with reference No. #LTO-688427953193. 

I would like to request for a Certification that a Complaint was filed by the Operator of CIGNAL Medical Clinic in Oroquieta City and that the same was received by LTO Regional Office X. I would like to be apprised of the action taken towards said complaint and the date the same was received as well as the date when of whatever action was taken relative to said complaint.
