Good day.

I would like to ask if it is possible to request for the LTO Certification notarized by the DFA online or if it is possible to request for it through a proxy? I will use this to apply for a license in Canada. I am already in Ontario, so I will not be able to apply personally in Manila.

Hope they can give me instructions on how to obtain the certification. Thank you!

Your Request

You asked for Driver's License LTO Certification notarized by the DFA.

Response to Request

Your FOI request is approved. I enclose a copy of all of the information you requested.

1.Here are the requirements in getting Driver's license certification:
 
-Photocopy of Driver's License and Official Receipt
-For DFA Authentication, the Driver's License should be valid
-Letter of Authority and Valid I.D. of authorized representative.
 
Kindly send your authorized representative, a copy of your driver’s license, authorization letter and a copy of a valid government issued ID of your representative, a corresponding fee of Php 100.00 is charge for the said certification. This certificate may apply at the License Section, LTO Central Office, East Avenue, Quezon City.

Your right to request a review

I would like to post a follow-up question:

Is it possible for me to request a digital copy of the application forms for the following (if there are any)?
1. Driver's license certification application form
2. DFA Authentication request form

Is this request offensive?
