Hello Sir/Ma'am,

I'd like to select/choose which accredited clinic is suitable for me based on my location and personal preference. I'd also like to make sure that I'm on a clinic/hospital that is accredited for getting medical certificate. 

I want to make sure that my payment goes to the government and not to any nefarious people.
