I would like to request for my serial number at the back of my card (driver's license). My card has been lost but i have acquired an Affidavit of Loss.

Your Request

You asked for Serial number at the back of my Drivers License.

Response to Request

The Serial Number is required in creating an LTMS Account. Unfortunately some of our clients still don't have a Driver's License Card or are still having trouble entering their serial number. So, in Creating your LTMS Account, you will see the first question will be: 
" Do you have a Philippine Driver’s or Conductor’s License?"

Kindly select "NO" and then proceed to complete the Client Information and continue with your renewal.

Your right to request a review
