Your request You asked for Drivers Licence Card Serial number Response to your request While our aim is to provide information whenever possible, in this instance this Office cannot provide you the information you are requesting. The LTO abides by the Data Privacy Act. Personal information cannot be publicly-published using the FOI Platform. However you will need to file a duplicate drivers license, what you will need is: 1. Duly accomplished Application for Permits and Licenses (APL) Form 2. Presentation of original Affidavit of Loss / Mutilated ID 3. Submission of one (1) photocopy and presentation of original valid ID with photo and signature You may file them at: 1. Licensing Centers / Licensing Extension Offices 2. Authorized District Offices / Extension Offices 3. Central Office - License Section Please be informed that LTO located on Malls are not authorized to duplicated driver’s license.
