Hi! Would like to know the process if LTO OR on Driver's License has been lost. It will supposedly be used to convert Philippine DL to Malaysian Driver's license. Or will Driver's License suffice?

Response to your request

While our aim is to provide information whenever possible, in this instance this Office cannot provide you the information you are requesting. The LTO abides by the Data Privacy Act. Personal information cannot be publicly-published using the FOI Platform.
Maari po kayo magrequest ng duplicate para sa nawalang lisensya. Ito po ang mga requirements for duplicate of license:
* Duly Accomplished form for Driver's License (ADL)
* Proof of Identity
* Affidavit of Loss
Magtungo lamang sa tanggapan ng LTO para sa pag-request ng duplicate driver's license. Application for duplicate of license po ay nagkakahalaga ng PHP 225.00.
