Response to your request

While our aim is to provide information whenever possible, in this instance this Office cannot provide you the information you are requesting. The LTO abides by the Data Privacy Act. Personal information cannot be publicly-published using the FOI Platform. Here are the requirements in acquiring a OR/CR:

Initially, you need to secure a notarized affidavit of loss CR/OR. Secondly, you have to go to your Originating District Office and present the said affidavit of loss. You will also be required to present 2 valid IDs for identification purposes. The Originating District Office shall be the one to issue you a duplicate CR/OR.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to LTO Assistant Secretary Atty. Teofilo E.
