Hi, 

Regarding above subject, I would like to know if there is a document that LTO issues which shows driver's license history from initial issuance up to latest renewal. This is requested by Japan licensing office from me to proceed with my license conversion. I already submitted to them the certification with Apostille but it only indicates the year of initial issuance and ask for above document.

Hope you could give me idea on which document to request from LTO and how to proceed.

Response to your request

You may apply for Driver’s License Certification / Driver’s License History at the License Section, LTO Central Office, East Avenue, Quezon City.

Kindly send your authorized representative, a copy of your driver’s license, authorization letter and a copy of a valid government issued ID of your representative, a corresponding fee of Php 100.00 is charge for the said certification. For more information you can send email to LTO Central Office License Section (co.license.section@lto.gov.ph)
