May 18, 2022 Dear Noel Jr; Greetings! Thank you for your request dated Apr 27, 2022 09:17:11 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Drivers License. Response to your request While our aim is to provide information whenever possible, in this instance this Office cannot provide you the information you are requesting. The LTO abides by the Data Privacy Act. Personal information cannot be publicly-published using the FOI Platform. However, here are the following are the requirements and procedures to wit: 1. Affidavit of Loss; 2. Law Enforcement Clearance, LTO-Regional Office ( if the application is outside Metro Manila); 3. Filing of Certification of License Records-LTO, License Section, East Ave. QC / any LTO Agency. 4. Duplicate and renewal (if the driver’s license is renewable) of driver’s license at the LTO – Agency, transact anywhere if previously automated. Please take note that the renewal of driver’s license is simultaneous with the application of duplicate driver’s license. Licensing Fees varies, it depends on the expiry date & the date of application with LTO. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to LTO Assistant Secretary Edgar C. Galvante at lto.cdmpao@gmail.com. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011). Thank you.
