Hi, I would like to request for a Certification of Completion from the Land of Transportation, that I passed and completed the actual driving test and written exams given by the said government agency, from when I first took it which was November 02, 2006. 
How do I go about it?

You or your authorized representative may apply at the License Section, LTO Central Office, East Avenue, Quezon City.
 
Kindly send your authorized representative a copy of your driver’s license, authorization letter and a copy of a valid government issued ID of your representative, a corresponding fee of Php 100.00 is charged for the said certification.
