Dear Madam/Sir,

May I request your good office of the attendance sheet of Atty. Arjay Puyot of PAO QC? This is to clarify some doubts caused by his inconsistencies.

I was requesting him for authorization letter so I can verify our appeal at the post office which hasn't been delivered yet. It's been more than two months already. My request was since October but his reason is he's on leave. Last week, he advised that he's on leave starting Monday until Tuesday this week. But last Friday, I followed my instinct to make a surprise visit in PAO QC and there I saw him assisting at the help desk.
