Good day! I would like to request the correction of my birthdate in my vaccination record. The vaxcert record of my birthdate shows January 7, 1999 instead of July 1, 1999. 

Also, I would like to request an update on my record as my second dose is still not showing. I got my second dose last July 23, 2021.
