Hi, Good morning!

I would like to humbly request for a financial assistance on my mother's hospitalization bill and medication expenses.

Currently, she is confined and has undergone major surgery called TAHBSO. She was diagnosed with Endometrial Adeno Ca which leads into bleeding last year and has decided to remove the tissues developing internally.

We hope you could grant our humble request so as to ease our burden in these tough times.

Specifically known as ASAP (At Source Ang Processing). This program aims to facilitate
accessibility in the filing and processing of request for Individual Medical Assistance
Program (IMAP) for indigent patients confined in the affiliated hospital. Patients NEED
NOT TO GO TO PCSO office to file their request for assistance. They can immediately file
their request and submit the necessary requirements at the PCSO Help Desk.
