I would like to request for a Certified copy of (Decree of Entitlement and Distribution) of the late SPO2 Wilfredo M. Arevalo dated September 12, 2012. We lost the original copy way back December due to Typhoon Odette and we need it to renew my mother's membership in AFPSLAI. Thank you and more Power!

While our aim is to provide information whenever possible, in this instance this Office does not have [some of documents and]* the information you have requested because DPRM has no jurisdiction on what you are requesting for. However, you may wish to contact <PNP RETIREMENT AND BENEFITS ADMINISTRATIVE SERVICE  (PRBS)> at < local 7551>, or resubmit your request addressed to the correct agency on the eFOI portal at <foi.dpcr.gov.ph>.
