I am writing this letter to ask about my request for change of last name and civil status.

I have submitted the Endorsement for change of last name and civil status including original copy of Marriage certificate with OR at ISPPO Ilocos Sur but my online PAIS is not yet updated. May I ask about the update of the said request/endorsement

While our aim is to provide information whenever possible, in this instance you can ask the PAIS section of your respective Region/Station regarding on your request.  however you already submitted all the necessary documents for your CDLB.
