This is with regards to my father, PSUP ARNOLDO OUANO TARONA (Badge # 0-03558) who died last October 03, 2021.

I am processing the filing of Transfer of Pension to my mother, ANTONIA C TARONA.

IRT to the above, I am requesting for the following:

1. Computation of Lump Sum/Retirement Gratuity

2. Lump Sum Voucher

3. Certificate of Last Payment
