Thank you for your request dated Apr 04, 2023 02:18:58 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Blotter Copy of incident Report.

While our aim is to provide information whenever possible, said copy of Incident Report can be easily  requested to the place of occurrence with proper procedures to follow. However, the concern person who are involved in the incident may go to Cauayan City Police Station, Cauayan City, Isabela to get a copy of excerpt blotter of the incident report.
