Dear SATURNINO DELUPIO,

In order for us to process your request for reprinting of ITR, kindly submit the following requirements personally to PNPFS Withholding Tax Section: (1) Affidavit of Loss, (2) Unit Endorsement, and (3) Photocopy of PNP ID.

Please state the calendar year of the BIR FORM 2316 that you are requesting.

Moreover, we would like to emphasize that PNP Personnel should request such documents on their respective BFO.


During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Apr 22, 2022 03:38:10 AM, your request will be closed.
