You asked for Loss passport.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not issue police report. Moreso, the requested police report is acquired through personal request in the nearest police station where your passport was lost. However, you may wish to contact or visit the San Fernando Police Station in SACOP Brgy Maimpis, City of San Ferndnando, Pampanga to personally report the lost passport and eventually request for police report.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to PRO3 FOI Officer at pro3.rpcrd.pnp@gmail.com.
