PSA issued Birth Certificate of Patrolman/Patrolwoman Applicant 
Photo copy of Valid ID;
PSA issued Birth Certificate of dependents;
PSA issued Certificate of Non-Marriage (CENOMAR)
College Diploma;
Transcript of Records;
Good Moral Character (from school)
Eligibility: Report of Rating
Clearances: National Police Clearance; Barangay Clearance; Mayor’s Clearance; Prosecutor/Fiscal; MTC; RTC; and NBI
Whole body picture; 2x2 ID picture with name tag (use white background)
Height waiver, if any
