REQUIREMENTS:
1. Civil Service Personal Data Sheet (Revised 2005).
2. PSA-authenticated Certificate of Eligibility;
3. Authenticated Certificate of Eligibility;
4. Two (2) pcs. 2x2 colored black and with white background picture with name tag, 1 bust-size picture and 1 whole body picture;
5. Authenticated Transcript of Records/Diploma;
6. Local Clearances such as Barangay, Police, RTC/MTC and NBA;
7. Certificate of Good Moral Character (Issued by College/University); &
8. Two (2) Valid Identification Cards
