This is Hannah Dorissa Teloza Hertez, daughter of PO3 EDUARDO BAÑOLA HERTEZ with Badge Number 049449, requesting the service records of my Dad. He is currently and terminally ill and he advised us to process his exit and clearance. We really don't know where to start but my dad was been part of EPD, Mandaluyong City.
