Good morning sir/ma'am. I am Harvy Kay B. Tocyapao, a contractor. I would like to request for a copy of the spot report of a shooting incident on July 21, 2022. I am applying for the temporary suspension of a project from the DPWH and one of the requirements is the spot report and certification to justify the peace and order situation.
