Dear Sir/ Madam,

Hi my name is Ma. Cristina D Golingan. (DOB October 8, 1975)  I have taken the NLE in Iloilo City year 2007 (June and December) 
May I request for a certification that I have applied and have completed all the requirements to qualify to take the Nursing Licensure Examination.  May I please also request for certification that if I wish to take the exam now, that I will be eligible to do so as per the current requirements.
