Good day! 

Good day!

I would like to get an authenticated copy of my Certificate of Registration since I really need it for my job application. I've been requesting the said document since 2017 but I never received a copy of it. I also filed another request last January at your office in Pagadian City but so far, there are no updates regarding my request. 

I am hoping that you will finally grant my request in order for my application to continue forward.
