Hi Mam/Sir, 

May I ask I can request a Board Certificate (Lupon) for Nursing Board through this channel? Let me know what details and requirements are needed from me. 

I have not so far claimed my Board Cert since passing the Board exam. I hope I can still get an original copy. I got my (PIC) license on May 08 2009.
