Hi. I am Chito Fernandez Jr. I am a Registered Nurse with 0525429 Registration Number. I am trying to have a State boar Certification but an error message shows that I have record mismatch. Probably because before my suffix JR was always together with my first name when I apply however now these days there is a specific area for name suffixes in any type of registration.

My PRC Identification has a First Name CHITO JR; While when I register in LERIS, I input my JR into the Suffix Box.

Also the Middle Name is Suñiga on my ID while in LERIS is SUNIGA.

Please help me fix my issue.
