Hi! I would like to ask for assistance in my prc account due to record mismatched. I cannot make any appointment due to record mismatched in my given name. There's a missing period (.) in my given name in the verification of rating.

Kindly provide and send to prc.archivesandrecords2020@gmail.com your profession, license number, a valid ID (can be a PRC license), PSA (birth certificate), Notice of Admission(for initial registration and e-oath transactions) and the email address you have used for the PRC account for verification purposes. Indicate your FOI tracking number as email subject.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sep 21, 2022 08:02:22 AM, your request will be closed.
