Good day Sir/ Madam,

This is to seek for your kind assistance for the retrieval of PRC Leris account. I have failed to recall the email address and the password that I have used in applying for my examination last 2018. Now, I am not able to file a schedule online for my license renewal.

I am aware that this situation was brought by shortcomings and so I am humbly asking for your help regarding this matter.
