My petition for the change of surname due to marriage was already approved last August 2023. However, when I tried to request for a duplicate of my license, it says, record mismatched since the name that appears in my LERIS is still my maiden name.
I am sending this message to request a change of name and status on my online profile/leris account so I can proceed with request for a duplicate before this year ends.
