To whom it may concern, 

Good day! I would like to request for the following information:

1. What are the usual reports related to medical malpractices
2. Annually, what are the average rate of such reports
3. What are the common reason of the malpractice eg. wrong procedure or diagnosis
4. Does the hospital become liable on this or it's purely a medical professional case?
5. Statistics per illness/ medical procedure
