Greetings!

Thank you for your request dated May 11, 2022 08:50:42 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Request for board certificate (lupon).

Response to Request

Both the PRC ID and the COR(board certificate) are usually issued after the oath-taking ceremonies.
NOTE: No COR and PIC shall be issued to any successful examinee or applicant admitted for registration without examination unless he/she submits proof of having taken the Oath of Professional.

In case you haven't received on the day of oath-taking ceremony , you can still claim the PRC ID and COR at the PRC office/branch you originally selected when you set an appointment for initial registration. 

For application of duplicate copy, below is the procedure and requirements.
Procedure:
Step 1. Accomplish this Action Sheet (AS).
Download Action Sheet for Registration Certificate Replacement.
Step 2. Pay prescribed fees at the Cashier.
Step 3. Present duly accomplished AS together with the Affidavit of Loss/Damage and receive claim slip at: Window 16, Window 18 and Window 30
Step 4. Claim your documents as scheduled. Please refer to your claim slip for further instructions.

Requirements:
Duly accomplished Action Sheet
Affidavit of Loss/Damage
Official Receipt
Passport size picture with name tag in white background

Good day, sir/madam!
I have registered already po last November 2021. Is there an update po regarding my board certificate (lupon). Because those who passed this January 2022 already have their lupon. Tha k you so much.
